Job Description
Job Summary
The Social and Health Equity Navigator will assist members in addressing social conditions that impact health outcomes, assist with the development of a process to collect, analyze, and report data metrics, and ensure these cost-effective services are provided to members who need them the most. The Social and Health Equity Navigator will also work collaboratively with other departments to identify population SDOH needs and work to help find solutions via partnerships with community organizations or other agencies.
Our Passport Health Plan by Molina Healthcare works closely with local providers and community advocates to improve health outcomes in the Commonwealth of Kentucky. We deliver government-funded, high-quality health care to approximately 324,000 Kentucky Medicaid members (as of January 2021).
For these positions we are seeking a candidate with community based work experience. A candidate with an active and unrestricted Community Health Worker Certification (CCHW) preferred. Candidates must have a valid driver’s license with good driving record, proof of insurance, and be able to drive within applicable state/locality with reliable transportation. Bilingual-Spanish speaking candidates encouraged to apply. Further details to be discussed during the interview process.
Remote position with 50%-70% travel to members home, provider office, and community visits.

Location Role 1: Louisville, KY (Jefferson County) or surrounding county (Oldham, Shelby, Spencer, Bullitt, Nelson, Hardin, or Meade)
Location Role 2 (Perinatal Focused): Louisville, KY (Jefferson County) or surrounding county (Oldham, Shelby, Spencer, Bullitt, Nelson, Hardin, or Meade)
Work hours: Monday through Friday 8:30 AM to 5:00 PM EST with flexibility to accommodate member needs.

Job Qualifications
Required Education:
Bachelor’s Degree in public health, social work or behavioral sciences or an equivalent education and demonstrated experience in the social determinants of health
Required Experience:
Preferred Education:
Master’s Degree in public health, social work or behavioral sciences or similar discipline
Preferred Experience:
Preferred Licensure/Certification:
Licensed in Social Work, Counseling, or other related field
PHYSICAL DEMANDS:
Working environment is generally favorable and lighting and temperature are adequate. Work is generally performed in an office environment in which there is only minimal exposure to unpleasant and/or hazardous working conditions. Must have the ability to sit for long periods.
